In-Home Aged Care Services in Brisbane

Aged Plus provides empathetic, professional aged care services in Brisbane that help you or your loved ones maintain independence at home.

What is the Support at Home Program?

The Support at Home program is the new, simplified way for older Australians to access government-funded services at home. Launched on November 1, 2025, it replaces the old Home Care Packages (HCP) and Short-Term Restorative Care (STRC) programs with a more flexible, needs-based system.

Our Core Services

When you choose Aged Plus you choose a compassionate and human-centred approach that values clarity over complexity and a warmer, more personal approach. Our team helps you to navigate the sometimes confusing details of in-home care in Brisbane, avoiding the usual pain points. We’re an aged care provider assisting seniors, their families, their carers and their GPs, and our Brisbane aged care services include:

Personal Care & Grooming

Personal care involves tailored in-home assistance to help seniors maintain high standards of hygiene and dignity. Aged Plus provides support in all kinds of personal care areas from general grooming to medication and nutrition.

Domestic Assistance

Domestic assistance helps seniors stay comfortable in their homes and maintain their quality of life. Sometimes a little assistance with daily household tasks can go a long way, and it’s that kind of household assistance that Aged Plus provides.

Social Support & Companionship

Staying connected with a supportive community is important for seniors. It promotes physical and mental health and reduces stress and loneliness. Aged Plus can provide personalised social support, both group and individual, that meets the needs of seniors. This may be company or transportation to special events or understanding one-on-one companionship.

Transport & Community Access

Transport and community access connects seniors with door-to-door assisted transport, allowing them to access the local community and its services. Aged Plus provides this extremely beneficial service to seniors who struggle with driving and public transport, helping them attend appointments and socialise.

Meal Prep & Nutrition

Meal prep & nutrition is an area that covers services like meal planning, cooking and delivery to make sure seniors receive a varied selection of tailored meals. Meal preparation can involve ready-made delivered meals, in-home food preparation and companionship and assistance while dining.

In-Home Nursing Support

Whether you’re recovering from surgery or require ongoing support for a health condition, our qualified registered nurses provide clinical levels of care in your own home. Adapting our Brisbane in-home care to meet your needs, we monitor your condition and respond efficiently and professionally.

About Our In-Home Care Solutions

In Australia, in-home care solutions are delivered through the Support at Home program, a new simplified approach that replaces the old Home Care Packages (HCP) and short-term restorative care (STRC) programs with a more flexible needs-based system. The new system comes with some important changes for families seeking aged care services in Brisbane:

Eight ongoing funding classifications
The new system replaces the old four levels with eight tiers to more accurately reflect the patient’s specific needs.

Quarterly budgets
Funding is now allocated every three months, meaning faster adjustments if your health needs change.

Faster access
The program aims to reduce wait times so you can receive the right level of support from the start.

Aged Plus takes a respectful, compassionate and human-centred approach to finding the right Brisbane in-home care solutions.
